Wellness Tier Eligibility

 

1. Active and Pre-Medicare member and spouse (if applicable) must have a physical completed by the due date of December 15, to enroll into the Wellness Tier for the following benefit year. 
2. A physical form must be completed and signed by the Physcian.
3. Form is not required if physical is completed at Union Trades Medical Center.
4. Form must be received by Local 38 Fund Office by December 15

 

This is an ongoing annual requirement to maintain Wellness Tier Status.

2026 Benefit Year

Physicals completed between 01/01/2025 - 12/15/2025 are eligible for the Wellness Tier for the 2026 Benefit Year.

 

Friendly Tips: 

• Medical Benefits operate on a Calendar Year basis. A physical can be scheduled any time after January 1st of the following year with no limitations.
• Physicals and labs/blood work completed at Union Trades Medical Centers are zero cost share to the member. 
• Schedule ahead of time at the Union Trades Medical Center. As the deadline of 12/15/2025 approaches, appointments will be booked up!
• Form is not required when utilizing Union Trades Medical Center.
